Please fill out the information below the best that you can. The more information that you can provide, the better we can handle the situation.

NOTICE:

Service Day: Every Wednesday

Time of arrival: Approximately 9 am

WORK ORDER DEADLINE: No work order will be accepted after 12:00 pm Tuesday, the day prior to regular service.

All work orders after the 12:00pm deadline will be added to the next Wednesday service.

All information requested with the RED *asterisk is required to be properly filled out.

Failure to do so will stop this work order from being sent.

This question is for the safety of your pet. Pets can get stressed at times when a stranger enters the home, plus with the pest equipment, it only causes more stress.

Addition Information About Pest Location, such as Kitchen-behind fridge., PLEASE do not say EVERYWHERE -because they are not - we need to know where the pest are observed.

I give permission for Florida Termite and Pest Control Tech to enter my unit to perform pest control. A duplicate key of your unit is located in the main office. The entry key will be checked out and checked back in when service is complete. *

Without permission granted by you, we cannot enter your unit. Please be sure a "working" entry key is available in the office. If your key is "not working" then we cannot service your unit at the present time and you will have to resubmit a work order for the following week or another time.

I want a pest free home, I want a the service below. Our service protects your home from general household pest such as ants, cockroaches, pantry pest, occasional invaders, etc. The fees listed DO NOT COVER Fleas, Ticks, Bedbugs or Rodents.

One Time Service - 30 Day warranty Only - $70.00
Monthly Service - Pest warranty for service to service - $30.00
Quarterly Service - Pest warranty from service to service - $55.00
Once a Year Service - Pest warranty year round - $210.00
Fleas - $150.00 - This treatment requires 2 treatments @ $75.00, generally 2 weeks apart. Total payment is due upon first service. - See Flea prep list on the Waterside Pest Page. If your unit is NOT properly prepared, your service may not be performed.
Ticks or Bedbugs - Fees are handled on a severity/time/material basis. Each job is different.
Rodents - Fees are handled on a severity/time/material basis. Each job is different.

If you have an interior pest issue (not termites) one of these boxes must be selected. If not selected, your work order will be rejected. If unsure call our office at 727-855-7555

It is advised to be at your unit during the time we are on property, we always try to get to everybody in a timely manner. BE ADVISED, not all treatments are the same and may take longer than others, Please be patient.
